Identity evidence

UK Passport Details Form (Download 73KB, opens in a new tab) - If you haven't included your passport details in your application you’ll need to send in a UK Passport Details Form.

If you don’t have a valid UK passport, you’ll need to send in your original Birth or Adoption Certificate or non-UK passport instead.

Changes to your application

CO2 Form (Download 997KB, opens in a new tab) - If there’s been a change in your circumstances you must notify us as soon as possible as this can affect your entitlement.

Tuition Fee Loan Request Form (Download 89KB, opens in a new tab) - If you need to change the amount of Tuition Fee Loan you’ve requested, you’ll need to complete this form.

Confirmation of benefits
