If you’re unable to use our online service you can use these forms to apply for student finance or provide information in support of an application. Please download and save forms before you print. Some browsers don’t print the forms correctly when printed from a browser.

Applying for specific financial support if you have children, adult dependants or a disability

If you’re applying for help with childcare costs

If you have completed the Part-time Grants for Dependants form (PTGFD) you should also complete and return this form:

Birth certificates or adoption certificates for evidence

If you don’t have a valid passport, you’ll need to send in your Birth or Adoption Certificate instead. You also need to complete and return this form:

This form contains two separate declarations. The Applicant's Declaration of Identity must be signed by you, the student, to confirm your identity. The Identity Confirmation Form is to verify your identity and must be completed by a person of good standing in the community who has known you for at least two years.

Please note: this person must be currently in employment, and they must supply a phone number for their place of work.

Confirmation of benefits

If you or your partner receive certain state benefits and are unable to send evidence ton confirm these with your application, you’ll also need to complete this form:

Change in your circumstances

If you need to tell us about any change in your circumstances through the year, for example a change in your course or the university or college you’re attending, you’ll need to complete this form:
