Forms and guides for academic year 2009/10 are available from academic year 2010/11.

UCAS application numbers

UCAS have recently changed their processes for academic year 2009/10 onwards. They no longer provide the UCAS application number to students, simply a Personal ID. Our online application only asks for the Personal ID. However, we do ask for the application number on the paper forms. The forms are already printed for 2009/10. Please be assured that students’ applications will not be held up as a result of not providing the UCAS application number. We will amend our paper application forms from academic year 2010/11.

2009/10 Full Time Students

Students who have already applied for support and wish to amend the loan amount they requested should complete either:

Birth Certificates or Adoption Certificates for evidence

We recommend that new students applying for support in 2010/11 submit their passport as proof of identity. Those students who are not passport holders can submit their Birth or Adoption Certificate, but must also complete and return the Birth/Adoption Certificate Declaration Forms (bilingual) (152kb).

This form contains two separate declarations. The Applicant's Declaration of Identity must be signed by the student to confirm their identity. The Identity Confirmation Form is to verify your identity and must be completed by a person of good standing in the community who has known you for at least two years. Please note: this person must be currently in employment, and they must supply a phone number for their place of work.

Disabled Students Allowance

If you have completed or are completing your application for student finance and you also want to apply for Disabled Students' Allowances, you should complete the slimline version of:

If you are eligible for DSA, Student Finance Wales will send you a letter confirming which of your expenses have been approved, so that you can be reimbursed. Your expenses may include travel, books in Braille and accommodation. You should use the following form to claim these costs back:

Parents and partners of students

If you are giving financial support to a student you may require these forms:

To provide your financial information in support of a student's application (where it has not been provided on the main application form)

If your household income for the financial year 2009/10 will be at least 15% less than it was in financial year 2007/08, you should complete the following form and send it to Student Finance Wales with evidence.
